I have been working with this skilled nursing facility over the past few years, and have really had good communication recently with the staff between our residents in their care from our assisted living community. Melanie and those working with her to get people through their rehabilitation and back home are doing a great job. We appreciate your partnership and care, residents return at their best possible level of independence.

I am primary care physician in the area and had a patient and good friend pass away unexpectedly at Grand Boulevard. I tried to talk to the staff and politely find out what exactly happened but Healther Hanna, the director of nursing is very unhelpful and even hung up on me. Such behavior in unacceptable. The patient signed a special release that I should have access to records, I even offered to have family sign a release of records. However, she never gave a release of records form. I even told the family that they might need to sign a release of records but we never got a form. It has been a week now Heather Hanna says it has to go through legal. We could have done that a week ago but she is purposely hiding something. Such secrecy generally means that they did something that caused my patient and friend to pass away. Very unacceptable.

Not a fan of Grand Boulevard Rehab upon arrival no photo/ decor on walls in rooms, must wait for someone to put in a code to let you out, and doors locked at 7! I have visited the Manor at Bluewater Bay on several occasions and it is immaculate. Dinner is served hot not cold. This place is a joke to be sitting on Grand Boulevard in Miramar. The physical therapy room is not much bigger than my kitchen, and the food square is the size of my master bath! For the amount of money people pay here per month this joint should be top notch. It's unbelievable to know it not a great place ?. Insane asylums don't have art on walls. Note if you visit the door plates say .. knock this is my home!! Add some decorative art and make the rooms enjoyable for residents instead of just paint.

My very best friend is currently in this center and has been bitten by some kind of bugs ...and had to be moved to another room . But still to date we have not been notified or addressed from the facility to the family..I make weekly visits..I went today and no one said anything to me about it good or bad..My best friend was visiting her one night and noticed the bugs ..pictures were taken according to my friend..WHERE IS THE COMMUNICATION WITHE THE ADMINISTRATION ON THIS ISSUE AND STAFF. MAKING WONDER ABOUT THIS FACILITY...
