About Rosewood

24 Hour Customized Living Services for seniors at Health Providers, Inc.are services offered to residents 55/65 or older. Elderly individuals on the Elderly Waiver may also take advantage of these services. This gives residents a chance to live in a wonderful home like environment with services suited to your needs. The homes are set up to resemble a home much
like yours and mine. All staff that work at Health Providers are given the necessary training to complete and provide personal care to all people who live at the homes. Health Providers, Inc. will provide laundry-wash, dry and put away clothing, meals-three per day, housekeeping-clean bedrooms, vacuum, dust, change linens, empty trash, bathrooms, routine activities, health and
hygiene, medication set up/administration/insulin dependent, medication review, transportation to local appointments, socialization, and self administration of drugs. We will provide three meals a day, specific to the individual diet, afternoon and evening snacks, 24-hour supervision, assistance
with grooming, bathing and dressing as needed per assessment. These services are included in the monthly service rate, which is based on the monthly service caps given by Minnesota Department of Human Services. Room and Board costs are designated by Chippewa County Family Services.
Consumers that live in our homes must not require 24-hour skilled nursing care. Activities are done throughout the week to promote socialization. Staff will also assist residents at shopping areas, out for coffee, or whatever they may like to do. Health Providers, Inc. will work with Hospice and other outside agencies to ensure the resident has all support services necessary. Health Providers, Inc. has staff sufficient in qualifications and numbers to adequately provide the services agreed to for the Class F License. Health Providers,
Inc. also is registered as housing with services agency and has an Adult Foster Care License through the County.
